Are you a compassionate and dedicated Community Psychiatric Nurse (CPN) seeking a rewarding opportunity in Worcester? We are currently recruiting for a motivated individual to join our team in providing holistic mental health care within the community setting.

Location: Worcester, United Kingdom

 

Responsibilities:

  • Conduct comprehensive mental health assessments and develop individualized care plans for patients.
  • Provide therapeutic interventions and support to individuals with complex mental health needs.
  • Collaborate closely with multidisciplinary teams, including psychiatrists, social workers, and other healthcare professionals, to ensure coordinated care.
  • Offer counseling, advocacy, and education to patients and their families to promote recovery and self-management.
  • Monitor and evaluate patients' progress, adjusting treatment plans as necessary to achieve positive outcomes.
  • Facilitate access to community resources and support networks to enhance patients' social integration and well-being.

 

Requirements:

  • Registered Nurse qualification with current NMC registration and a valid CPN qualification.
  • Minimum of 1 year of experience working as a CPN in a community mental health setting.
  • Strong assessment, communication, and interpersonal skills.
  • Ability to work autonomously and collaboratively within a team environment.
  • Empathy and understanding towards individuals experiencing mental health challenges.
  • Commitment to providing person-centered care and promoting recovery-oriented practices.
